Once we’ve reviewed your form, we will reach out to discuss the next steps. If you choose to proceed with an appointment, we will send you the complete new patient packet, which includes medical history forms, consent documents, and other necessary paperwork.
Please be aware that our schedule often fills up months in advance, though availability can change. If necessary, we will gladly add you to our waiting list after your appointment is scheduled.
If your insurance requires prior authorization, please ensure it is obtained before scheduling your appointment.
