Dear Patient,
It is with mixed emotions that I am announcing my retirement from Northwest Houston Neurology, PA. It has been a great pleasure providing for your health care needs over the years and it is not easy for me to give up.
My last day with Northwest Houston Neurology will be May 26th, 2022. As you all know, this practice is staffed with very knowledgeable and reputable neurologists. I highly recommend that you select one of these physicians for your future neurology medical needs.

Please note that patients under the age of 18 will need to transition to another practice as our other physicians are not accepting patients under the age of 18. The only exception to this is for children ages 6 and up with sleep or seizure disorders.
We understand that some of you may want to select a new physician outside of our practice. Please be assured that my staff and I will do everything we can to make the transition smooth and stress free. I recommend that you begin looking for a new neurologist as soon as possible. You may need to contact your primary care provider or your insurance plan.
Your medical records are confidential and will remain on file with Northwest Houston Neurology. If you choose another physician within our organization, you do not need to take any action concerning your records. If you choose a physician outside of our clinic, you will need to sign an authorization form before we can release your records. Please note, by law, we cannot share your medical information without your consent. You may sign this form through our office or you may complete this form at your first visit with your new doctor and they can request the records on your behalf.
